This program is designed for people with prediabetes to cut their risk of developing type 2 diabetes through lifestyle changes such as healthy eating and physical activity.
Stronger Families, Stronger Futures
This program connects pregnant people and parents with young children to resources to help families thrive.
Medical Home Program
This program focuses on building strong partnerships between families and their pediatric care providers.
Putting on AIRs Program
This program compliments the Medical Home Program by providing in-home asthma education and management.
Connecticut Early Detection and Prevention Program (CEDPP)
This program empowers women with the knowledge and tools to reduce their risk for heart disease and stroke
