The Community Health and Well-Being (CHWB) program is a cornerstone of Trinity Health Of New England's integrated, people-centered healthcare system. We recognize that health goes beyond just medical care. Our mission is to address the physical, spiritual, and behavioral health needs of residents throughout Connecticut and Western Massachusetts.
What Makes Us Different
Many factors influence health, such as access to transportation, housing, food security, safety, education, and language services. Unfortunately, traditional healthcare often focuses solely on clinical care, neglecting these vital social determinants of health. Here at Trinity Health Of New England, we're different.
We believe in addressing all factors that impact well-being by integrating social care with clinical care. Recognizing that clinical care alone accounts for only a fraction of what determines health, we address social determinants like housing, food security, and transportation that directly affect overall well-being.
Our Approach
Our Community Health and Well-Being (CHWB) team works with community partners and healthcare providers to connect individuals with essential services, helping to overcome barriers and improve health outcomes. Through this integrated approach, we support healthier individuals and communities.

Through innovative programs, referrals, and service coordination, our CHWB team works to:
- Empower individuals: We equip people with the resources they need to manage their own health and well-being.
- Strengthen communities: We collaborate with our communities to address systemic challenges affecting health outcomes.
- Promote self-sufficiency: Our goal is to help individuals and families thrive – not just survive.
- Focus on vulnerable populations: We recognize that social and clinical care are deeply interconnected, and that both are essential for optimal health, so we prioritize those facing poverty or other vulnerabilities
