PGY2 Pharmacy Program

PGY2 pharmacy residency programs build on Doctor of Pharmacy (Pharm.D.) education and PGY1 pharmacy residency programs to contribute to the development of clinical pharmacists in specialized areas of practice. PGY2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care or other advanced practice settings. Residents who successfully complete an accredited PGY2 pharmacy residency are prepared for advanced patient care, academic, or other specialized positions, along with board certification if available.

Background

Our organization has been incorporating pharmacists into a variety of practice settings and precepting pharmacy learners for over 30 years. We have offered an ASHP-accredited PGY1 residency since 2000. We were the first site in the state of Connecticut to offer an ASHP-accredited PGY2 in Ambulatory Care. Many former residents continue to practice at Saint Francis and currently serve as preceptors.

The Saint Francis Hospital Anticoagulation Service has provided pharmacist management of anticoagulation for over 20 years. Pharmacists are respected members of inpatient, ambulatory and emergency settings, and pharmacists with expertise in thrombosis and hemostasis management are integral parts of these teams. Our program has the support of University of Connecticut clinical faculty with expertise in thrombosis and hemostasis as well as research and teaching opportunities with University of Connecticut esteemed faculty.

In 2024, Saint Francis Hospital received the TRAIN grant from the Anticoagulation Forum to develop the Thrombosis and Hemostasis Management PGY2 residency program and is currently in pre-candidate status. The program is being funded and developed to address a need for pharmacist experts in thrombosis and hemostasis management as these high-risk disease states and medication therapies are complex and increasingly result in adverse patient outcomes. Expertise in this area will afford residents opportunities in a variety of practice settings. Our goal is to address the individual needs and interests of our residents while adhering to the principles and philosophies described in the ASHP Accreditation Standard for Postgraduate Year Two (PGY2) Pharmacy Residency Program.

PGY2 Thrombosis and Hemostasis pharmacy residents can practice at the top of their license, managing a wide spectrum of disease states. Rotation structures vary from an interdisciplinary team approach to independent medication management under collaborative drug therapy management agreements. Residents will have frequent contact with nationally recognized experts in thrombosis and hemostasis management via bi-weekly seminars with the other PGY2 Thrombosis and Hemostasis residency sites.

Upon completion of the Saint Francis Hospital PGY2 Thrombosis and Hemostasis Residency Program, residents will:

  • Have developed specialized clinical skills in the areas of thrombosis and hemostasis management
  • Be prepared to be leaders in anticoagulation stewardship
  • Have acquired skills to manage a transition of care service 
  • Be certified in Anticoagulation Management
  • Be prepared for a variety of clinical and/or academic pharmacy roles

Learning Experiences:

The resident will complete at least 12 months of experience, including an orientation block, the ASHP Anticoagulation Certificate, required block and longitudinal rotations, and elective block rotations. The resident will participate in bi-weekly educational seminars with Anticoagulation Forum experts and other PGY2 Thrombosis and Hemostasis residency sites. The resident will complete a major project and present posters at national pharmacy and anticoagulation conferences.

Schedule

Residents will be on campus for rotations Monday through Friday. The emergency medicine block requires evening and weekend hours. Staffing is required one half day per week in the Anticoagulation Service. Four weeks per year are allocated for research and scholarship associated with research projects.

Professional Meetings and Conferences

At a minimum, residents will attend and present at the American Society of Health-System Pharmacists (ASHP) Midyear Clinical Meeting and either the Anticoagulation Forum’s National Conference on Anticoagulation Therapy or the Thrombosis and Hemostasis Summit of North America (THSNA).

Applicant Requirements:

  • Be a graduate of an ACPE-accredited college of pharmacy
  • Completion of an ASHP-Accredited PGY1 Residency Program
  • Obtain licensure in the state of Connecticut by September 30th of the residency year
  • Eligible to work in the state of Connecticut for the entire duration of the residency year
