Welcome to your NEW MyChart Patient Portal

Trinity Health Of New England has transitioned to a new patient portal – Trinity Health MyChart – to better meet your needs. Any new information from visits to a Trinity Health Of New England doctor’s office or hospital from November 2, 2024 and beyond, including prescription refills, visit notes and lab results, will now be in your new MyChart patient portal.

Some information prior to November 2, 2024 may not be available in your new account due to differences in the versions.

You will continue to be able to view your old data in your former MyCare/MyChart account. Your providers will always have access to your historical medical data.

Access to our previous patient portal, will end on May 29, 2026.

This portal contained historical information for visits prior to TogetherCare go-live (Trinity Health’s Epic instance). Patients can continue to access their health information on our current portal, Trinity Health MyChart.

Access to Historical Information & Release of Information

  • Patients who need copies of their complete medical records, including information not available in Trinity Health MyChart, Trinity Health’s standard Release of Information (ROI) processes remain in place
  • Patients should contact their local Health Information Management (HIM) department to request records
  • Providers will always have full access to historical patient data for continuity of care
