Trinity Health of New England can accept specialty referrals through any method that is most convenient for your practice. Once received, our staff will contact your patient within 5 business days to schedule their appointment.

• Phone: Call our Central Referrals team at (833) 296-8669
• Epic: Send via TogetherCare.
• Direct Message: Select desired specialists Direct Messaging address through your EMR if you have Direct Messaging capability.

Please ensure the following information is included in your appointment request:

• Specialty
• Patient demographics
• Insurance information
• Referring provider, practice name, phone and fax numbers
• Diagnosis/reason for referral
• Insurance referral if required
• Most recent office note
• Labs, diagnostic studies, and pathology if clinically applicable
