Online Bariatric Support Group - December

Thank you for your interest in the Online Bariatric Support Group. Below is some important information and ground rules for participating in the group. Please review the information and reply “Yes” if you agree or “No” if you decline and do not wish to participate:

Any information you hear or see through your participation in the support group is not to be shared. Participants in the support group depend on other participants to maintain confidentiality. By participating in the support group, you agree not to use or disclose any information learned through the group. You also agree that the support group is not a treatment service.

Participation in the support group is voluntary. By participating, you acknowledge that your name, email address, image, or other identifiable information my be visible to other participants. You agree not to use any information obtained about other participants outside of the group. 

IMPORTANT: By using the collaboration technology for participation in a group session you are sharing your sensitive personal health information (PHI) with participants and via cloud technology. Your PHI will be viewed via the internet and Trinity Health cannot assure that your PHI will not be accessed, viewed or obtained during a session or following a session. Please note: The collaboration technology provider and your health care provider use safeguards to PHI; however, unauthorized persons may find a way to join a session and any participant may withdraw PHI during the session. Further, when you access the session you may be connecting in a manner that would not prevent unauthorized persons from joining and viewing the session. As complete confidentiality cannot be assured, please use care in choosing to participate, accessing the session and sharing sensitive information.

If you agree to this, please select “Agree” to receive invitations to the support group meetings. If you do not agree, you may select “Decline.” You may opt-out of receiving support group invitations at any time by contacting the support group coordinator to request to opt-out.
