Local Grant Strengthens Saint Mary’s Medical Home Program

Ives Bank awarded Saint Mary’s Hospital $2,500 in renewed grant support for its Medical Home Initiative patient fund. The renewed support strengthens the program’s patient fund and helps expand services for more than 300 children and families across 42 towns in Northwestern Connecticut.

The Saint Mary’s Medical Home Initiative provides comprehensive, hospital-based care coordination for children from birth through age 21 who are living with chronic physical, developmental, behavioral or emotional conditions. A dedicated team of experienced nurse care coordinators leads this work, drawing on decades of pediatric expertise to guide families through complex care needs. They partner closely with families to develop individualized care plans, connect them with medical and community resources and advocate for services that are often not fully covered by insurance.
By coordinating care across health care, education and community systems, the Saint Mary’s Medical Home Initiative reduces barriers and improves outcomes for children and their families. This investment from Ives Bank helps ease financial strain, expand access to critical support and ensure families receive the compassionate, comprehensive care they need to thrive.
