On Wednesday May 21, the Rotary Club of Waterbury generously donated $750 to support Saint Mary’s Hospital’s Case Management Department. Case managers assist patients by coordinating services such as transition of care, health and wellness referrals, disease management, behavioral health, and chronic condition improvement programs.

“The generous donation from the Rotary Club will help the Case Management team support patients who have barriers to discharge, such as lack of transportation, a need for durable medical equipment and even medications,” said Lori Therrien, RN, Manager, Case Management, Saint Mary’s. “We are truly grateful for this wonderful gift that will go directly toward caring for some of our most vulnerable patients. 

The Rotary Club and Saint Mary’s share a mission in providing care for the common good. “The Rotary Club of Waterbury donates money annually to non-profit organizations that provide services and care to the underserved of the Waterbury community,” said Mary Zasada, Rotary Club President and Health Informatics Specialist at Saint Mary’s. “We have included Saint Mary’s in this group for the last five years.”
