Saint Francis Leads Connecticut with Lowest Readmission Rate for Heart Failure Patients

Over a three-year reporting period, Saint Francis Hospital led the State of Connecticut in achieving the lowest readmission rate for heart failure patients, based on data provided by the Centers for Medicare and Medicaid Services (CMS). Between July 1, 2021, and June 30, 2024, Saint Francis ranked first out of 26 hospitals statewide that reported and was the only hospital with a better average than the national rate. With its “Better than Average” numbers, Saint Francis ranked 49th nationally among over 3,200 hospitals across the country.
Saint Francis has been nationally recognized for outstanding care for heart failure patients.
Per CMS data, Saint Francis had a readmission rate of 17.4%, better than the National Readmission Rate Average of 19.7%. The Heart Failure Program at the Hoffman Heart and Vascular Institute has earned both national and regional recognition for excellence in patient care, achieving certification in Advanced Heart Failure by The Joint Commission. This certification recognizes hospitals that have achieved high standards for the treatment of heart failure patients. In addition, Saint Francis has consistently received American Heart Association’s Get With The Guidelines® Gold Plus Awards for Heart Failure, including in 2025.
Approximately 6.7 million U.S. adults are living with heart failure, a number that is expected to increase to more than 8 million by 2030, according to the AHA. Despite the name, heart failure doesn’t mean the heart has stopped working, but that the heart is having a hard time pumping blood and oxygen throughout the body. However, patients with heart failure can live longer and more active lives if they are diagnosed early and establish a treatment plan with their care team.
The Hoffman Heart and Vascular Institute uses the most advanced diagnostic technology to evaluate the underlying cause of heart failure, which allows providers to create customized care plans to manage all stages of the condition. The comprehensive, multidisciplinary approach helps patients and families navigate the full spectrum of treatment.
