Shayne Sampognaro Named Colleague of the Month at Saint Francis

Shayne, Transition of Care Pharmacist, was selected as the honoree for December.

In his nomination, Shayne’s manager wrote that he, “has been a shining example of all Saint Francis values and has made a lasting impact in the transition of care space for all patients and for our clinical team.” Shayne, who serves as the lead for the Meds2Go program, has received multiple shout-outs from clinicians and patients alike for expediting the discharge process, assisting patients with difficult medication acquisition issues, and ensuring safe medication discharge practices. 

Shayne is also recognized for developing and leading multiple population health initiatives that identify patients at high risk of readmission and solve key medication-relation problems prior to discharge and safely into the outpatient setting. Shayne’s manager noted, “He is a friendly face on all care floors, with nurses always appreciative of his Meds2Go enrollment and kid patient counseling he offers with his learners. He embodies the key values of Saint Francis as a person, and, as a result, our patient discharge process is safer with him on the team!”
