Trinity Health Of New England Leaders Participate in Training with Press Ganey

On August 19 and 20, leaders from across Trinity Health Of New England gathered on the Saint Francis Hospital campus for a transformative two-day Cause Analysis Training, facilitated in partnership with Press Ganey. The 16.5-hour course marked a significant step forward in the region’s commitment to patient safety and system-wide learning.
Leaders from around the region took part in a two-day Cause Analysis Training, facilitated in partnership with Press Ganey.
Designed for physicians, nurses, and quality and patient safety professionals, the training introduced a methodological framework for Root Cause Analysis (RCA), equipping participants with tools and techniques applicable across departments and care settings. The curriculum emphasized:
- Human performance in complex systems
- Fundamentals of Cause Analysis, including deep dives into individual and system failure mechanisms
- Event and Causal Factor Charting
- Health Equity Contributors and their impact on patient experience and outcomes
The training also reinforced CEO and senior leader ownership of RCA and Apparent Cause Analysis (ACA) processes, while building unit-level skills to analyze minor and precursor events. A key highlight was the integration of an equity lens into event analysis, aligning with Trinty Health’s broader mission to improve outcomes for all patients.
Participants had the opportunity to engage with experts from Press Ganey and System Office, fostering collaboration and energizing teams through shared learning and engagements.
As the organization continues to standardize RCA practices across ministries, this training will serve as a foundational milestone in elevating a culture of safety, accountability, and continuous improvement. There will be another training session at Mercy Medical Center on September 30 and October 1. Future training sessions will be scheduled for Executive Leadership Teams and event managers who conduct front line investigations.
