The Saint Francis Hospital Community Health and Well-Being team recently hosted the first group of patients enrolled in the Hypertension Management Program, designed to support community members with uncontrolled high blood pressure. The initiative provides blood pressure monitoring, personalized nutrition education and coordinated care.

Service partners, including the University of Saint Joseph Health Coaching Team, Wilson-Gray YMCA, and Harvest Meadow, joined Saint Francis team members from Community Health and Well-Being to provide education, including food prep demonstrations and tastings led by Jaime Foster, PhD, RD, Program Coordinator of Clinical Social Care. Participants learned about the DASH diet – Dietary Approaches to Stop Hypertension, a heart-healthy, low-sodium eating plan focused on fruits, vegetables, whole grains and low-fat dairy.

The event was a great success, thanks in part to lead organizer Gillian Walcott, a Community Health Worker at Saint Francis.
